There is one small detail not documented on that page:

The YR command should include the initial blob sent by the client (NTLM 
Negotiate packet). For historical reasons the intial implementation in 
Squid did not but that is another story.

Also, to comply with MS SSP design requirements it is sane to assume KK 
may result in yet another TT if additional information is required from 
the client. This won't happen for the NTLM protocol, but maybe for the 
Negotiate protocol?
