My experience (from doing it manually) is that upgrading directly from
12.04 to 14.04 is broken. I ended up re-installing from scratch.

You might want to try 12.04 -> 12.10 -> 13.04 -> 13.10 -> 14.04.

Or re-install from scratch...

Might be that Ubuntu also know this and so aren't offering a direct
upgrade from LTS -> LTS (I haven't had a good look around for evidence
of that, though).
