I've had similar issues the last few weeks - different carrier. Using
another carrier and the same modem I've had no problems... (bit early to
blame the carrier just yet though).

Is this a wireless internet or cable?

dmesg?

anything interesting in /var/log/?

If wireless:-

enable debug for pppd

then:-
grep -e pppd /var/log/debug

also:-
ifconfig ppp0

what sort of modem?
does it use /dev/ttyUSB or other device?

sigmon for the relevant port?
