Often alsa mutes everything by default, this could be the issue.

Try running:

for x in Front Master PCM Surround Center ; do amixer set $x 100% unmute
; done

Add any other channels you have - i.e. sometimes sound comes out Front
instead of PCM and no matter how much you unmute and boost PCM you get
nothing ;)
