We're generating linked/indexed PDFs using Latex (we haven't tried *between*
document links, but can't see why it would be any different to the URLs we embed).
The source text would need to be suitably marked up of course (so whatever
you use to generate your .tex knows when to link, index etc.).

OO files are gzips containing the document content as XML and some
style instructions (not that I have confirmed this for myself yet).
We have been having lots of fun with Cocoon to convert XML to targets
including HTML, although Cocoon might be overkill for you.
