you need the macserial module compiled, so remove support for standard generic
serial ports and have the macintosh serial module. (macserial)

PowerMac Z8530 serial driver version 2.0
tty00 at 0xc9a8e020 (irq = 22) is a Z8530 ESCC (cobalt modem)
tty01 at 0xc9a95000 (irq = 23) is a Z8530 ESCC (IrDA)

so the modem is on ttyS0

there is however an issue with how long it takes to power the modem up when
you use it. When I have it in the pon scripts I need to have the init string
ATX1, without it it wont work when I pon ispname.
