[PATCH] Improve hot record detection in CTDB

Hi,

CTDB currently identifies hot records based on hopcount which is not the
appropriate metric for measuring contention.  A better metric for record
contention is number of migrations per second.

These patches implement hot record identification based on migrations per
second.

Please review and push.
