some fixes for mod_ntlm_winbind

Hi folks,

I think I've cleaned up the coredump problem with mod_ntlm_winbind,
plus a silly assumption or two I made in the Apache 2 code. Patch
attached. I've tested against IE, Microsoft Messenger (!), curl, and
Mozilla, and it seems pretty solid both on Apache 1 and Apache 2.
