I can only remember 1 vacuuming bug (which possibly could cause this)
that *may not* be fixed in 4.10.x (depends on version and distro
patches). This bug is:

I suggest that you check the source to see if the relevant patches have
been applied.  However, this bug is rare and you appear to be seeing it
a lot.

The previous links I provided summarise everything I know about this
issue.  I encourage you to read them carefully, see if you can find a
bug, and hopefully contribute a patch.  We have previously audited the
code to attempt to find such bugs, concluded that out-of-memory was the
only likely scenario, and then found appropriate messages in the logs
to support this.  You should probably take a similar approach.

I would also suggest lots of careful testing as you upgrade.
