[Samba] NT_STATUS_ACCOUNT_LOCKED_OUT

Hi,

Not sure if this is relevant or not, but we often get 
NT_STATUS_ACCOUNT_LOCKED_OUT when windows uses a saved password somewhere.

Check this:

and remove (if any) relevant saved passwords, to be sure that this is 
not what is causing your issue.
