Try removing uidNumber and just use uid. Here is a section from my
slapd.conf file.
index objectClass
eq
index cn,mail,givenname,sn,displayName
eq,subinitial,pres
index uidNumber,gidNumber,memberUID,member,uniqueMember eq
index uid
eq,subinitial,pres
