[PATCH] vfs_ceph fix fallocate and ftruncate handling

Hi,

The attached patchset adds a missing .fallocate_fn hook to vfs_ceph
and fixes "strict allocate" handling. A couple of minor cleanups
are tacked on to the end of the series.

Feedback appreciated.
