[PATCH] Fix INSTALL_DIR() to respect $DESTDIR and log

INSTALL_DIR() currently creates directories in their final destination,
ignoring $DESTDIR.  Make it respect $DESTDIR and log what it is doing.

Also:

* Make CTDB use INSTALL_DIR() instead of equally broken install_dir()
* Drop unused, broken install_dir()

Please review and maybe push...
