[PATCH] Drop CTDB tunable option NoIPHostOnAllDisabled

This is a couple of minor test fixes followed by removal of the
NoIPHostOnAllDisabled tunable.  The new default is simply to host no IP
addresses when all nodes are unhealthy (or disabled).  This obeys the
principle of least surprise - we've had many users confused when
unhealthy nodes host public IP addresses.

Please review and maybe push...
