[PATCH] CTDB tool/test improvements

This started as unit tests improvements for the ctdb CLI tool, in
preparation for pushing Amitay's replacement tool.  The main goal was
to add a lot of new test-cases.

It has grown to additionally include:

* Some documentation fixes

* ctdb CLI tool simplifications and improvements

* WHATSNEW.txt update

This is quite big but only touches ctdbd in one fairly trivial way, so
it quite safe.

This branch includes numerous patches from Amitay, which are already
marked as reviewed by me.
