[PATCH] CTDB IP allocation cleanups

This removes some unnecessary complexity from the CTDB IP allocation
code.  I added all of the code now being removed in the 3rd commit to
avoid assigning IPs to nodes that are not in RUNNING runstate.
Instead, those nodes now report no available IPs.
