[PATCHES] CTDB standalone build against Samba's lib/util and lib/tdb_wrap

This patchset removes CTDB's local lib/util and lib/tdb_wrap
directories... and fixes a few little things along the way.

To make the transition to samba-debug easier, the CTDB log ringbuffer is
removed.  As far as we know, nobody has used this for quite a while.

Please review and push if OK.

These patches are in my ctdb-build branch:

For context, I also have a ctdb-unified-build branch that follows from
this branch.  This includes changes required on top of ctdb-build to get CTDB
into a unified Samba build.  I'll post that once this set of patches is
in...  but feel free to take a peek.  :-)
