[PATCH] Re-submit macro removal and winbind pipe use for AD DC IDMAP

Jeremy,

Can you please push these for me?  I combine them with the reinstatement
of the AD DC IDMAP change from Volker, as it was based on that
originally, and the revert broke them.  As both need to go in, a rework
to do these in another order seemed like a waste of effort. 

Finally, it includes the removal of the old, incorrect default pipe
location that caused the trouble from the winbind_struct_protocol.h

As I've verified that the changes now work, I trust you have no
objection to them being merged into master?

removed, as we realised we need to coordinate those with OpenChange.
