Code works but I have two requests for a change.

Firstly, "bool dst_needs_rw" isn't clear enough.
It doesn't really articulate what the options
are here - they are really READ|WRITE, or WRITE.

There ought to be a better way to articulate
that (sorry, can't immediately think of that
now).

Second point:

is a fallthough on a switch statement without at least
a comment showing this is intended. That's a horror and
an abomination :-) :-).
