Samba 4 phpLDAP configuration removal

This is to remove the phpldapadmin.conf that is generated during provision
(and migration if I recall right, possibly other samba-tool functions). The
config that was generated was not able to be used without tweaking, and
even if you tweaked it to work, phpldapadmin didn't work quite right with
the samba 4 ldap. I don't remember what it was right off, but something
didn't work quite right.
