NT_STATUS_NONE_MAPPED failures

Someone recently was complaining that many things in the Samba 4.0 AD DC
failed with the error NT_STATUS_NONE_MAPPED.  I can't find the reference
right now, but I'm happy to dig into it if folks speak up again.

In particular, level 1 (at least) debug logs would help a lot - the
idmap code can give this error, but it's mostly when the rfc2307 entry
in AD isn't correct or consistent (ie has duplicates).
