It turns out that I applied your latest patches, but when I was
reviewing it I looked at an earlier version!

I now see all the TODO entries. For most of them I don't know the
answers immediately, but I think what you need to do is build up a set
of examples with the correct computed topologies so you can start
testing, and working out how to solve these problems from the results.
