I'm continuing to work with Howard Chu on this, and he will shortly have
a Virtual Machine that reproduces the problem as I see it.  

To be clear, what do you see the same when running:

TEST_LDAP=yes OPENLDAP_ROOT=/usr/local make test

This is the environment we are currently trying to fix, in the hope it
will then fix the real-world deployment issues.  Hopefully they are one
and the same thing (but they may not be - there may be multiple
different issues).
