[PATCH] Cleanup for Coverity issues

This patch mostly concerns a number of Coverity warnings about unchecked
return values.  I've been meaning to submit this for a while, sorry
about the delay.

 

Per a previous discussion on here, prs_init() is split off via a macro
so now there's a prs_init_empty() which does no allocation and therefore
can never fail.
