LDB work in my 4-0-local branch

Tridge,

I've pushed a few of the fixes (including a patch for this regarding LDB
DNs I had done a while back) to my 4-0-local repo.

(For the benefit of the list, tridge and I have been chasing down bugs
relating to us walking past the end of ldb_val strings).

My branch includes work to use the AD schema as the basis for indexes
and attributes, which uncovered this bug.
