Update to NTLM docs

Thankyou *very* much.  

This really is very interesting research, and in particular the worked
examples make this very, very useful to reference and to work with.

One little job I would love someone to do for me some day is to
integrate all these worked examples into the LOCAL-NTLMSSP test, to
ensure we don't break on basic cryptography, particularly with the
less-used options.
