Automatically Creating User Accounts

Hi,

I am interested in automatically creating a user account when an unknown
user id attempts to connect the first time.  Clearly, this could seem insane
to some!   But , I am seeking guidance on how to proceed with it and to know
from more experienced people whether it is possible or not.

Any pointers or suggestions are welcome.
