Done.  

Kai:  I'll take your sam_passdb stuff in as soon as you like - it's
easier to track if we keep it all in CVS.

(Yes, the bar on 'must be fully functional' is lower while the SAM is
being developed - but once we start hooking into the rest of Samba I'll
get very strict, on both new and existing code...)
