I'm not sure, but I think that it starts from oplock problems.
I was able to get the following messages with relatively small log at debug
level = 10 (Ok, it is 1.8M ;).
It was clean started smbd, and the client was accessing two ".xls" files.
Although I didn't get tdb corruption, I always had these troubles befor it happened.
The full log for machine "misieknew" is available from:
http://orchid.cbk.poznan.pl/~vela/log.olaf.bz2

And is about 57KB big.

Example:

[2002/03/14 17:53:17, 0] smbd/oplock.c:oplock_break(843)
oplock_break: client failure in break - shutting down this smbd.

and

[2002/03/14 17:53:17, 3] smbd/server.c:exit_server(492)
  Server exit (oplock break failure)

And in smbd log three minutes after I see something like this
(I don't know if it is related to this case or no):

[2002/03/14 17:58:19, 5] tdb/tdbutil.c:tdb_log(475)
  tdb((null)): tdb_brlock failed (fd=9) at offset 4 rw_type=1 lck_type=13
[2002/03/14 17:58:19, 5] tdb/tdbutil.c:tdb_log(475)
  tdb((null)): tdb_brlock failed (fd=10) at offset 4 rw_type=1 lck_type=13

The full log is on:
http://orchid.cbk.poznan.pl/~vela/log.smbd.bz2

On the client side Win2kSP2 PL:
Excel hangs for about 30 seconds, and then I get (translated from Polish):
"Access denied for 'filename'. The file can be read-only or you try to
access read-only site. Or the server on which the documents are stored is not
responding"
And buttons 'Retry', 'Cancel'.

Hope it helps.
