i have created pdb_init_(sam/smb) and pdb_sam_to_smb() and
pdb_smb_to_sam().  a more advanced form of pdb_smb_to_sam() - probably a
different function - will at a later date fill in the blanks, by reading
lp_xxx() default values, presumably.

you can now remove the local getldappwent() functions and create... i
know: i'll do it in passdb.c, ok?
