Producing LM/NT Hashes

Thanks to all who responded to this.  I got lots of helpful input, but
wanted to specifically thank  Anton Roeckseisen
<Anton_Roeckseisen at genua.de> who sent code that compiled almost
out-of-the-box to do exactly what we needed; it takes a cleartext password
on stdin and produces LM and/or NT hashes on stdout.
