Log:
Remove the _krb5_rd_rep_type hack, and try to merge at least some of
our GSSAPI (to support DCE_STYLE) changes into the form of the
mainline code (indentation etc).

Add better error-case cleanup, and factor out a bit more of the code
into the _gsskrb5_create_ctx helper function.
