Merge from 3.0:
ntlm_auth:
 - add --require-membership-of
 - add and use new function for decoding hexideciaml-encoded strings
   into DATA_BLOBs
 - ntlm_auth now requests 'squashed' error codes

elsewhere:
 - don't dereference potentially NULL pointers in ntlm_check.c
