Aha.  So this is what I was seeing - can I get some more details?  I was
getting an access denied message in the AUTH2 response as opposed to the
TCP session being disconnected.  There was also an error in the event
log.

This was with APPLIANCE_HEAD (yeah yeah) and NT4sp6 PDC.  It was easy to
reproduce by running smbclient -L //foo -U administrator%password in a
tight loop in a couple of xterms.
