Added all spooler built-in forms. Msoft defines spooler built-in forms in a specific order.
The DEVMODE 'papersize' member variable is an array index to the entries in this structure.
Note: NT4 defines the forms "Letter" through "German Legal Fanfold", and W2K adds the rest.

printing/nt_printing.c: add forms to default_forms, mod nt_printing_init() to add all forms
  to ntdrivers.tdb, cleanup get_ntforms(), add_a_form(), delete_a_form(), and update_a_form()
