Added Luke's book to the list.

Unfortunately, Macmillan's web site is fairly poor.  The link to MTP was
broken when I checked, and the whole thing is javascript based.  Ick.  I
found no way to link to the page describing the book.  I have sent a short
whine to their webmaster, but it's not likely that I'll hear back.
Webmasters tend to know better than their customers. ;)
