Log Message:
Added in the missing RPC_ALTCONT rpc calls. JF - please look at this
change as it is cleaner than what you put into HEAD. The p->hdr struct
already contains the info needed to determine what kind of rpc packet
is incoming, no need for an extra parameter to a bind function.
Also added more debug for fault pdu reports.
