A large round of performance improvements:

  - When enumerating domain users and groups, fetch small chunks of data from
    the server and feed it back to the client.  Previously we fetched the data
    all at once and then fed it back to the client in small chunks.

  - Open sam handles to trusted domains only when absolutely necessary.

  - Improvements for handling domains with many (>10,000) domain groups

Group membership and caching of enumerated users and groups is currently
disabled.  Will get working again soon!!
