--log-file-format seems to be ignored?

You don't show the _exact_ commands you're using, so it's hard to tell
what the problem is.  Try not to obfuscate details unless you really
have to, as details can be very important...

Are you talking to an rsync daemon? If not, rsyncd.conf is not relevant.
If a daemon is being used when the rsyncd.conf should be in /etc/.
