One other possibility occurred to me:  if rsync encounted some kind of an error
reading the source files (such as a directory that it couldn't open), it will
output this warning "IO error encountered -- skipping file deletion".  Perhaps
that is the root of the missing deletes?  If so, you can specify --ignore-errors
to tell rsync to proceed with the deletions anyway.
