There are very likely holes in almost any backup strategy, but here's
the way it was done at the NAA's digital archive when I was there...
Two separate systems, each with its own raided disk array. Two
different operating systems - one Linux, one Windows. Both systems
contain the same content.
A tape library with a set of tapes in it, a set nearby ready to swap
in, and a third set off site - all rotated weekly. The set in the tape
library is not swapped out until a test restore of some of its
contents passes an integrity check.
There are still plenty of ways this can go wrong. That's why you
employ skilled admins.
