It depends. If the driver is in the Linus kernel, see the MAINTAINERS file at
the root of the source distribution; that should list the appropriate contact
details for the file.
Otherwise, for an out-of-tree driver there is no standard; you get to do
whatever sounds like a good idea, or they write down.
A final fallback is to contact the subsystem mailing list (eg: the DVB drivers
mailing list; see [1]), or the general Linux kernel mailing list, and ask for
help finding the upstream authors.
