are you running a transparent proxy by chance?  You might need to check 
your config.  I had a problem similar to this when using squid and 
iptables for a transparent proxy.  It was fixed by changing options like 
httpd_accel port or redirect_rewrites_host_header or 
httpd_accel_uses_host_header.  Not much but it may point you in the 
right direction.
