Connection Is Lost!!

Still having quick disconnect problems and would like to hear ideas on where 
to look for solutions.  Here is a summary of yesterday's session...

The above was taken from /var/log/messages    AFTER the session.

I did NOT voluntarily close the session.

What can I do better in future??
