[distcc] Support for user configurable IO timeout (PATCH)

Hi!

This patch adds support for a user specified I/O timeout (environment
variable DISTCC_IO_TIMEOUT). This patch updates both code and
documentation. The patch ist against version 2.18.3.

I'd be happy to see this integrated into an upcoming release of distcc.
