sam.ldb error

  Try something like that:

ldbsearch -H sam.ldb | grep -i pool


You should see something like this:

rIDAvailablePool: 1501-1073741823
rIDPreviousAllocationPool: 1001-1500
rIDAllocationPool: 1100-1599
rIDUsedPool: 0

Also give me the highest rid of your provision (it's the last 4 numbers 
in sid, so basically you get all you sid and you sort them).
